Tirzepatide, explained: one molecule, two receptors

Tirzepatide is usually described as the compound that does two things at once. That is accurate but unhelpfully vague. What follows is what the molecule actually is, why it behaves the way it does, and where it sits in regulation.

What it is

Tirzepatide is a synthetic peptide of 39 amino acids with a molecular mass of roughly 4,813 Da. That makes it about fourteen times the size of a small tripeptide like GHK, and a little larger than the 31-residue GLP-1 analogues it is usually compared against.

Its backbone is derived from GIP, glucose-dependent insulinotropic polypeptide, rather than from GLP-1. This is the detail that most summaries skip, and it matters: the molecule is a GIP peptide that has been engineered until it also fits the GLP-1 receptor, not a GLP-1 peptide with GIP activity bolted on. Receptor studies reflect that heritage. Willard and colleagues characterised it in 2020 as an imbalanced dual agonist, with affinity at the GIP receptor comparable to native GIP and weaker affinity at the GLP-1 receptor than native GLP-1.

Why it lasts for days rather than minutes

Native GIP and GLP-1 are both cleared in minutes. Two modifications change that.

Aib substitutions. At positions 2 and 13 the molecule carries α-aminoisobutyric acid, a non-natural amino acid. Position 2 is where the enzyme DPP-4 cuts native incretins. Aib has no hydrogen on its alpha carbon for the enzyme to work with, so the cut does not happen.

A fatty diacid. A C20 diacid is attached through a linker to the lysine at position 20. This chain binds reversibly to serum albumin, the most abundant protein in blood. A peptide bound to albumin is too large to be filtered by the kidney and is shielded from proteases, so it is released slowly back into circulation. The reported half-life is around five days, which is what turns an interval measured in minutes into one measured in days.

Both tricks are now standard in this class. Retatrutide uses the same albumin-binding approach to reach three receptors instead of two.

How it compares

Regulatory status

Tirzepatide holds a marketing authorisation in the UK and is marketed under the brand name Mounjaro. That makes it a licensed medicine when it is supplied as that product, through that supply chain, with that packaging and patient information.

Research-grade tirzepatide is the same chemical entity and a different product. It is manufactured to a research specification, it carries no marketing authorisation, and it may be bought, sold and held in the UK for laboratory research only. What matters for research supply

The checks are the ordinary ones, and they are worth doing in this order:

  1. Identity. The observed mass on the certificate should match tirzepatide, not a shorter analogue. How to read the certificate.
  2. Purity, with the method named. What a purity figure measures, and what it does not.
  3. Quantity. Purity is a percentage and says nothing about how much peptide is in the container. Ninety-nine per cent pure is not ten milligrams.
  4. Batch traceability — certificate, carton and batch lookup agreeing with each other.
  5. Storage2 to 8°C, dark, never frozen.
